Overview
DXM is a dissociative drug found in over-the-counter cough medicines that produces powerful psychedelic effects at high doses.
Dextromethorphan (DXM or DM) is the cough suppressant found in many over-the-counter cold and cough medicines. At doses exceeding the therapeutic range it becomes a powerful dissociative drug with psychedelic properties. Part of the morphinan class, DXM has sedative, dissociative and stimulant properties.
It is an NMDA receptor antagonist — similar to ketamine — creating a sense of distance from reality. Sold in syrup, tablet, spray and lozenge forms (e.g. Robitussin, NyQuil, Dimetapp, Vicks).
It is CRITICAL to ensure DXM is the ONLY active ingredient — other ingredients (antihistamines, paracetamol) can cause serious harm or death. There are four experience levels called plateaus based on dose in mg/kg. DXM is not to be confused with Dextrorphan or Dexamethasone.
Safer Using
CRITICAL: Only use preparations where DXM is the sole active ingredient — other ingredients can kill General rule: 1 plateau per week (3rd-plateau dose = wait 3 weeks; 2nd-plateau = wait 2 weeks) Use in comfortable environments with trusted people around you Beware of interactions — mixing with certain drugs can cause serotonin syndrome or severe ataxia Some people carry a CYP2D6 genetic mutation making DXM far more powerful — do your research Even after effects wear off in 4–6 hours, DXM stays active in your system for up to 24 hours
⚠ Avoid Combining With
DANGEROUS: MAOIs (extreme risk of death); Antidepressants (serotonin syndrome, potentially fatal); Diet drugs such as fenfluramine or phentermine (serotonin syndrome); MDMA (serotonin syndrome); Alcohol (potentiated CNS depression and blackout risk — avoid above 1st plateau); GHB/GBL (unpredictable, severe risk of unconsciousness and vomit aspiration); Opioids (CNS depression, breathing difficulty, hepatotoxic — very unsafe combination); Tramadol. CAUTION: Non-drowsy antihistamines (especially if already present in the DXM product); Benzodiazepines (potentiated sedation, loss of consciousness at high doses).
🚨 If Things Go Wrong
Roadside testing does not detect DXM, but police may conduct standard impairment tests. If someone becomes unconscious, place in the recovery position immediately to prevent vomit aspiration. Call 000 in an emergency.
Long-term heavy use is linked to persistent memory, attention and mood problems. Carrying drugs puts you at risk of trafficking charges — know your local laws.
Effects
Dissociation and out-of-body experiences Visual and auditory hallucinations Euphoria and mood lift Increased heart rate, blood pressure, sweating and itchiness Urinary retention, slurred speech Nystagmus (rapid eye movement), nausea, dizziness Short-term memory loss, confusion and disorientation Paranoia and altered time perception At higher plateaus: complete dissociation, near-death experiences, spiritual encounters Red/blotchy skin rashes, allergic-like responses
Dose & Duration
Ingested orally. Total Duration: 4–6 hours. Onset: 20–60 minutes.
Peak: 1.5–3 hours. Coming Down: 1–2 hours. Hangover: 1–12 hours.
WARNING: death has occurred at approximately 25 mg/kg. Plateau doses: 1st (1.5–2.5 mg/kg) mild stimulation; 2nd (2.5–7.5 mg/kg) intoxication; 3rd (7.5–15 mg/kg) dissociative; 4th (15–20 mg/kg) fully dissociative. Drug testing: Urine 1 hr–3+ days; Hair up to 90 days; Blood 15 min–2 days.
